Part-time hypnotherapy has been very successful for my clients for several years. Now, I find excuses to cancel appointments or find other more important things that have to be completed. Possibly plain old fear of failure as reared its ugly head. Any help would be appreciated.

Many factors to consider .. mostly related to satisfaction I suspect.
Financial.. perhaps you make more money more easily doing other things.
Emotional.. the effort of helping others with their problems is draining. The feeling that clients fail to make the effort to cooperate & help themselves, even a little.
Role changing.. you have to get of a role.. into another role & that takes time & effort..especially if cases are sprung on you without much notice.
The setup where you have consultations.. background noises & other distractions.
Pesonal life situation.. which may include your own relationships outside & inside the work environment.. attitudes & expectations of colleagues & clients, family & friends.
The "looking out the window" thinking/feeling that I'd rather be doing something else than all of this, signalling it is time for a change.. a holiday, a change of location.. something stimulating & interesting to do & learn.
